Focusing on chordomas and meningiomas, Dr. Al-Mefty has utilized his wealth of clinical experience to study and define the biological behavior of these tumors. Collaborative studies with cytogeneticists and pathologists exemplify the transitional research that led to clinical prognostic and management indicators.

His early interest in spondylotic myelopathy led to the recognition of intramedullary lesion that can be demonstrated by imaging. His animal research model establishes the pathophysiology of this baffling disease.

He focuses his effort in developing, teaching, growing, and organizing skull base surgery discipline. Consequently, he was attracted to the biological aspect of this disorder and made transitional research that shed light on the behavior and prognosis of some of these tumors.
